I've been everywhere oob there. Nothing happened :/
Haven't found a trigger for the freeing of Avina though
guess you need the quest for that... again...

Yeah, the last cutscene probably only plays if you activate the lever of her prison :/
Since Avina flies in the air in the PC version, I guess all other triggers are on a higher level as well

Without switching. I found a good spot to dash into the fountain from, which often gives you a big upwards boost (though you still most often don't go high enough :/)
So I'm afraid it's still pretty random
And yeah, I got sucked into the Roman fight that you would need to do on the way to the first Atrium
